Tommy Butler – Administrator

Tommy Butler is Onsite’s Administrator. He developed the Onsite Employee Health and Wellness Clinic in 2007 and has served as the Safety & Health Director for CMCSS since 1995.

Sharla Smith, RN – Clinic Manager

Sharla Smith has almost 27 years of experience at CMCSS. She is a Registered Nurse. Sharla has extensive experience in the Risk Management, Safety, & Health industry, and she has worked in various positions at CMCSS (within the same department) to include:   Assistant Risk Manager / Safety Coordinator, Safety & Health Manager, Nurse Manager, On-the-Job Injury Nurse Consultant, and School Nurse.  Prior to this she worked in the insurance field, and has maintained her Property and Casualty Insurance Producer license, to serve as a resource for CMCSS.

 

Tiffany Maxwell, LPN – Assistant Clinic Manager

Tiffany is the Onsite Assistant Clinic Manager and has been with Onsite since 2015. She previously worked in long-term care for 11 years. She has a son that attends CMCSS. In her spare time she enjoys spending time with her family and friends, watching her son in marching band, baking, and reading.

Kelly Foley, PA – Lead Physician Assistant

Kelly Foley has been Onsite’s Lead Provider since 2013. Prior to that he was a nurse for 12 years, with 8 of those years as a flight nurse.  In his spare time away from the clinic, he enjoys working on projects at home and studying theology. Kelly’s fun fact is that he likes to train horses in his time away from the office. Kelly takes special care of all of his patients.

Supervising Physician

Dr. Donald Wade Huffman – Supervising Physician

Dr Huffman specializes in Family Medicine and has been practicing since 1988. He attended medical school in Arkansas and completed his family practice residency in Ft Bragg, NC. He was previously in the military and landed at Ft Campbell. He has worked as a medical provider in Clarksville since 1996.

Nicole Loggins,  MSN, AGPCNP-BC
Nicole is a Board Certified Adult-Geriatric Primary Care Nurse Practitioner with a wealth of experience in various healthcare settings. She is passionate about helping people and serving the CMCSS/Montgomery County community. Nicole remains approachable, teachable, and committed to providing the best care for her patients. She emphasizes the importance of ensuring that every patient and colleague feels seen, heard, and valued. 
Jessica Mosley Coleman, RN, BSN, MSN, FNP-BC

Jessica Coleman is a Board-Certified Family Nurse Practitioner, who began at Onsite in January 2019. Jessica received her Bachelor of Science in Nursing from Murray State University in December 2006. In May 2012, she completed her Master of Science in Nursing from Tennessee State University’s Family Nurse Practitioner Program. Jessica has a vast amount of experience caring for patients of all ages, from birth to geriatrics. Her professional career has encompassed a wide variety of specialties, to include Surgical-Oncology, Primary and Urgent Care, as well as Adult Health Clinical Instructor for Belmont University’s RN-BSN Program.

Jessica is one of two primary providers at Onsite who also provide infusion services to our patients. Jessica is a creative thinker, and she truly cares about providing quality service to all her patients. Jessica was born and raised in Clarksville and loves life on her farm with her 2 children and a variety of farm animals.

Kayla Shelton, FNP-BC

Kayla Shelton is a Board-certified Family Nurse Practitioner who joined the Onsite team in March 2018. In 2010, she completed her Bachelor of Science degree in Nursing at Austin Peay State University. She worked in Labor and Delivery, Postpartum, and Newborn Nursery with NorthCrest Medical Center for 4 1/2 years. In 2014, she completed her Master of Science degree in Nursing at Austin Peay State University. Kayla has been practicing as a Family Nurse Practitioner since 2015. Prior to joining Onsite, she worked with Premier Medical Group in their Family Walk-in Clinic. We are grateful for Kayla’s assistance with all of her patients. Her service results in many dollars saved for all Insurance Trust members!

In her spare time, she loves spending time with her husband, niece/nephews, traveling, watching Disney Plus, makeup tutorials, and reading. Kayla and her husband are Disney fanatics! A fun fact about Kayla is in 2009 she was on The Price is Right and she actually won the showcase showdown! When you visit, you will have to ask Kayla what one of the major prizes is that she won! Kayla is here to assist you for your needs!

Danny Anderson, FNP

Danny Anderson is a certified Family Nurse Practitioner who earned his doctorate degree in nursing practice from Western Kentucky University. He is a veteran, who served six years in the United States Army. His nursing career experience ranges in specialties from nursing education, neurological intensive care, emergency medicine, medical-surgical services, and now family practice.

Danny is passionate about empowering patients through education and providing an excellent healthcare experiences for every patient he sees. Danny’s friendly disposition and professional demeanor ensures that he will be an invaluable member of the Onsite team.

Jenni White Barnes, FNP

Jenni has been an RN for 19 years and has had the privilege of practicing as an NP since 2019. She has worked in primary care and urology. She was a student in the CMCSS district and currently has 2 sons at Montgomery Central Middle School. She enjoys doing anything crafty and bingeing her favorite shows/movies on Netflix. She is a rare Clarksville native and looks forward to caring for our community. 

Angela Blocker – Counselor

Angela is a dedicated professional with a profound passion for healthcare and education. With a rich and diverse experience spanning over 17 years in healthcare, she has consistently demonstrated her commitment to improving lives and fostering growth in others. Angela has a Master’s degree in Human Services Counseling with an emphasis in Marriage & Family. 

Her approach to counseling and human services revolves around fostering independence and resilience, equipping individuals with the tools they need to navigate life’s challenges. One of Angela’s greatest joys is helping and teaching others. 

JennyC
Jenny Cooke – PRN DNP, APRN, FNP-C
Jenny hails from Puerto Rico and completed her Doctor of Nursing Practice degree at East Tennessee State University in May 2021. Prior to this, she obtained a Master of Science in Nursing with a focus on Family Nurse Practice from Maryville University in 2017, and a Bachelor of Science in Nursing from Austin Peay State University in 2014. Her nursing journey began in 2002 when she became a License Practical Nurse after graduating from Altamaha Technical College in Jesup, GA.
Jenny  is a fluent Spanish speaker and is dedicated to enhancing healthcare accessibility and outcomes for the Hispanic community in Montgomery County. She is particularly committed to diabetes prevention and management.
Jessica Myatt – PRN FNP
 Jessica is a Family Nurse Practitioner that joined the Onsite team in May 2020. She graduated with her Bachelor of Science in Nursing at APSU in 2016. She has worked in both adult and pediatric ER. In 2020 she completed her Master of Science in  Nursing at APSU. Jessica is currently working as a provider at Centennial Children’s Emergency Department and works PRN at both Onsite and Doctors Care. She enjoys spending time with her family. She is married to her high school sweetheart and hey have 2 beautiful children. She loves God, enjoys the outdoors, going on walks, and making others laugh. 
Danielle
Danielle Schaaf – PRN FNP
 Danielle started her career as an active duty army nurse after graduating from Creighton University in 2007. She worked in a variety of settings to include to include intensive and burn intensive care. After the military she attended the University of South Alabama where she obtained her Master of Science in Nursing and her Doctorate of Nursing Practice. She has been a nurse practitioner since 2017 working in settings to include internal medicine, family practice and perioperative medicine, She is a member American Associate of Nurse Practitioners and the Sigma Theta Tau honors society.
Brittany Sauers – PRN FNP
Brittany graduated from APSU’s MSN program in August 2021 and started Onsite as a nurse practitioner soon after. She had done her clinicals here at Onsite while in school and created great relationships with staff which led her to come back as an employee. She also works as a clinical instructor/adjunct faculty for APSU’s BSN nursing program. Prior to becoming a nurse practitioner, Brittany worked as a RN in the emergency department for 9 years. In her free time she loves spending time with her husband and daughter. They enjoy being swimming, riding on their side-by-side, camping, being on the boat, really anything outdoors. In her spare time she also enjoys reading. 
Robin Black – PRN FNP
 Robin has been a board certified FNP for 34 years and a board certified PNP for 33 years. Her past experience includes Vanderbilt Pediatrics, Pediatric GI in Birmingham, AL, and working in rural healthcare in Erin, TN.

Mike Lombardo – PRN PA-C, BA-PA, MPAS
 Michael has been in Family Medicine since 1998 then advancing his career by earning his Master if Science in Physician Studies from the University of Nebraska Medical Center through the Interservice Physician Assistant Program in 2007. After nearly 24 years serving in the Army and working for Fort Campbell as an independent contractor he dedicated to retire. After a year, he still felt the desire to put his knowledge and compassion to good use and provide care for his new community network by serving those who serve Montgomery County.

Michael enjoys his time away from the office being a husband, father, and papa. Michael and his wife, Michele, have 4 children and share their home with 3 of them along with their 3 grandchildren. Their oldest son is serving in the Air Force overseas. He has always told his children that its great to be blessed but its even greater to be a blessing. Words he loves by through his faith as a Christian. He enjoys spending time with family, growing a garden, and hunting. He does add he would like to take up fishing if his busy schedule would ever slow down but for now he just loves being a servant of God. 

Susan Hankins – PRN FNP
 Susan has been in the medical field over 10 years. She has experience in the ER, urology, and urgent care. Susan is a native of Clarksville and graduated from CMCSS.  She enjoys spending time with her husband and three children. In her free time she loves to travel, read, hike, and plant.
Jennifer Stewart – PRN FNP
 Jennifer has been in the nursing field for 18 years, seven of those years are a Nurse Practitioner and 11 years as a RN in the ER and ICU. Arizona is her home state but she has lived all over since marrying into the military. Tennessee is their fifth duty station and they love it here. In her free time she enjoys painting, sewing, gardening, and reading.

Robyn Poole – Lead Medical Office Assistant
 Robyn has been with Onsite since 2022. She is driven by her desire to make a positive impact on the people around her. In her free time she likes to workout, spend time with her family, and hang out with her fur babies.  
Michelle Smith – Lead Nurse
Michelle has been with Onsite since February 2018 and was a former PRN CMCSS School Nurse.  In 2020, she helped to start the Onsite Infusion program, which has resulted in annual multi-million dollar savings.  She now participates in all aspects of the clinic with a direct focus on employee trainings, infusion, and Team Lead for all administrative matters.  In her spare time, she likes to spend time with her family, beach trips, and also crafting.
Megan Byrne – Infusion Coordinator
Megan is originally from Upstate New York. She moved here with her husband and 3 children in 2022. She has been a RN for 14 years; working in OB/GYN and Labor & Delivery. Megan enjoys walking, going to the gym, and spending time with her friends and family.
Robert Conley – Special Projects Manager
 Robert joined the Onsite Team in 2021 after completing his degree in Healthcare Management at APSU. He works diligently behind the scenes to ensure our clinics run smooth and efficient. He handles a lot of our equipment / product testing and logistical needs to keep our 5 clinics on the leading edge of patient care.

Chantelle Gonzalez, MA
 Chantelle has been a medical assistant since 2013. She has worked in different types of clinics. She grew up in a military family and has lived in Clarksville since 2003. Chantelle married her highschool sweetheart and together they have two amazing daughters. In her spare time she enjoys hang out with family/friends, playing video games, reading, and doing yoga.
Cheryl Hooton, RN
 Cheryl has been a nurse since 1999. She is originally from NH. She moved around with the military and landed in Clarksville in 2004. In her spare time she enjoys gardening and reading. 
Kelsey Keaton, MA
 Kelsey, a native of Clarksville, has served as a medical assistant since 2014, with expertise primary care and OBGYN. Beyond her professional pursuits, she cherishes moments with her husband and their two beloved dogs, Daisy and Ruger. Additionally she finds joy in attending church and engaging in lively pickleball matches with friends. 
Nina Hufford, MA
 
Courtney Pamplin, RN
 Courtney is a Clarksville native. She has been in a nurse since 2020. She is a cat mom of 2, Sailor Moo and Albrich. Her favorite time of year is fall and she loves all things Halloween. In her free time she enjoys thrifting and doing crafts.  Fun fact about Courtney: She lived in the UK for 4 years (2014-2018).
Samantha Petrone, LPN
 Samantha is a Clarksville native. She has been in the medical field for 16+ years. In her spare time she enjoys spending time with her 6 children, riding horses, working on cars, and riding 4-wheelers. 
Marteka Hall, MA
 
Marteka is a dedicated and hardworking individual with a strong background in the medical field.  With 11 years of experience, she has developed a solid foundation in her career, with 6 years as a Medical Office Assistant and 5 years as a Medical Assistant. She has 2 boys that attend CMCSS schools that keep her busy playing travel ball and for CMCSS.  In her free time, Marteka enjoys spending quality time with her friends and family, which is essential for maintaining a healthy work-life balance.
