Tommy Butler – Administrator
Tommy Butler is the Onsite Administrator. He developed the Onsite Employee Health and Wellness Clinic in 2007 and has served as the Safety & Health Director for CMCSS since 1995. Tommy holds an Occupational Safety and Health degree from Murray State University and he has served as the Administrator for Onsite since its inception.
Sharla Smith, RN – Clinic Manager
Sharla Smith brings over 27 years of experience with CMCSS and is a Registered Nurse. She joined Onsite in 2016 and has a deep background in Risk Management, Safety, and Health, having held various roles within CMCSS, including Assistant Risk Manager/Safety Coordinator, Safety & Health Manager, Nurse Manager, On-the-Job Injury Nurse Consultant, and School Nurse. Her extensive nursing experience, combined with her knowledge of risk management and safety, has made her a valuable asset in ensuring the well-being of both employees and students. Before joining Onsite, Sharla also worked in the insurance field and has maintained her Property and Casualty Insurance Producer license, serving as a knowledgeable resource for CMCSS.
Tiffany Maxwell, LPN – Assistant Clinic Manager
Tiffany is the Onsite Assistant Clinic Manager and has been with Onsite since 2015. She previously worked in long-term care for 11 years. She has a son that attends CMCSS. In her spare time she enjoys spending time with her family and friends, watching her son in marching band, baking, and reading.
Kelly Foley, PA – Lead Physician Assistant
Kelly Foley has been Onsite’s Lead Provider since 2013. Before joining Onsite, Kelly was a nurse for 12 years, with 8 of those years spent as a flight nurse. Kelly’s fun fact is that he likes to train horses in his time away from the office. He also enjoys working on projects at home and studying theology. Kelly takes special care of all of his patients.
Supervising Physician
Dr. Donald Wade Huffman – Supervising Physician
Dr Huffman specializes in Family Medicine and has been practicing medicine since 1988. He attended medical school in Arkansas and completed his Family Practice residency in Ft Bragg, NC. He was previously in the military and landed at Ft Campbell. He has worked as a Medical Provider in Clarksville since 1996.
Dr. Huffman has served as Onsite’s Medical Director since 2018.

Jessica Mosley Coleman, RN, BSN, MSN, FNP-BC
Jessica Coleman is a Board-Certified Family Nurse Practitioner, who began at Onsite in January 2019. Jessica received her Bachelor of Science in Nursing from Murray State University in December 2006. In May 2012, she completed her Master of Science in Nursing from Tennessee State University’s Family Nurse Practitioner Program. Jessica has a vast amount of experience caring for patients of all ages, from birth to geriatrics. Her professional career has encompassed a wide variety of specialties, including Surgical-Oncology, Primary and Urgent Care, as well as Adult Health Clinical Instructor for Belmont University’s RN-BSN Program. Jessica is one of two primary providers at Onsite who also provide infusion services to our patients. Jessica is a creative thinker, and she truly cares about providing quality service to all her patients. Jessica was born and raised in Clarksville and loves life on her farm with her 2 children and a variety of farm animals.
Kayla Shelton, FNP-BC
Kayla Shelton is a Board-Certified Family Nurse Practitioner who joined the Onsite team in March 2018. In 2010, she completed her Bachelor of Science in Nursing at Austin Peay State University. Kayla worked in Labor and Delivery, Postpartum, and Newborn Nursery with NorthCrest Medical Center for over 4 years. In 2014, she completed her Master of Science in Nursing at Austin Peay State University. Kayla has been practicing as a Family Nurse Practitioner since 2015. Before joining Onsite, she was employed with Premier Medical Group, where she worked at their Family Walk-in Clinic. We are grateful for Kayla’s assistance with all of her patients. Her service results in many dollars saved for all Insurance Trust members!
In her spare time, she loves spending time with her husband, nieces and nephews, traveling, watching Disney Plus, makeup tutorials, and reading. Kayla and her husband are Disney fanatics! A fun fact about Kayla is in 2009 she was on The Price is Right and she actually won the showcase showdown! When you visit, you will have to ask Kayla what one of the major prizes is that she won! Kayla is here to assist with your medical needs!
Danny Anderson, FNP
Danny Anderson is a certified Family Nurse Practitioner who earned his Doctorate in Nursing Practice from Western Kentucky University. He is a veteran, who served six years in the United States Army. His nursing career experience ranges in specialties from nursing education, neurological intensive care, emergency medicine, medical-surgical services, and now family practice.
Danny is passionate about empowering patients through education. He is committed to delivering an exceptional healthcare experience for every patient under his care. Danny’s friendly disposition and professional demeanor ensure that he continues to remain an invaluable member of the Onsite team.
Jenni White Barnes, FNP
Jenni has been an RN for 19 years and has had the privilege of practicing as a Nurse Practitioner since 2019. She has worked in primary care and urology. She was a student in the CMCSS district and currently has 2 sons at Montgomery Central Middle School. She enjoys doing anything crafty and bingeing her favorite shows/movies on Netflix. She is a rare Clarksville native and looks forward to caring for our community.
Angela Blocker – Counselor
Angela is a dedicated professional with a profound passion for healthcare and education. With rich and diverse experience spanning over 17 years in healthcare, she has consistently demonstrated her commitment to improving lives and fostering growth in others. Angela holds a Master’s degree in Human Services Counseling with a focus on Marriage & Family.
Her approach to counseling and human services revolves around fostering independence and resilience, equipping individuals with the tools they need to navigate life’s challenges. One of Angela’s greatest joys is helping and teaching others. Angela joined the Onsite team in January 2022.

Mike Lombardo – PRN PA-C, BA-PA, MPAS
Michael enjoys his time away from the office being a husband, father, and papa. Michael and his wife, Michele, have 4 children and share their home with 3 of them, along with their 3 grandchildren. Their oldest son is serving in the Air Force overseas. He has always told his children that it’s great to be blessed but it’s even greater to be a blessing- words he lives by through his faith as a Christian. He enjoys spending time with his family, growing a garden, and hunting. He does add that he would like to take up fishing if his busy schedule would ever allow him to slow down, but for now, he just loves being a servant of God.

Marteka is a dedicated and hardworking individual with a strong background in the medical field. With 11 years of experience, she has developed a solid foundation in her career, with 6 years as a Medical Office Assistant and 5 years as a Medical Assistant. She has 2 boys who attend CMCSS schools, and they keep her busy playing travel ball for CMCSS. In her free time, Marteka enjoys spending quality time with her friends and family, which is essential for maintaining a healthy work-life balance. Marteka has been with Onsite since 2023.

Charli has been with CMCSS since 2016. She comes with over 20 years of Administrative Assistant experience, a two-year teaching certificate, and an Associate of Arts degree. She has been married to her husband for 26 years. They have two children, a grandchild, and 2 dogs. She enjoys going to theme parks (especially Universal Studios Florida), camping, event planning, and anything Harry Potter!

Chasity has been with Onsite since 2022. She strives to ensure all of our patients have an exceptional experience while visiting our clinic. Outside of work, Chasity is a mother of three. In her spare time, she enjoys spending time with her family and friends.
