Important Policies:

  • All texts, emails, and telephone calls are a courtesy to the patient. The patient is always ultimately responsible for the scheduling / cancellation of their appointment. 
  • We ask that all patients arrive ten minutes prior to their appointment in order to complete any required paperwork. Please bring your insurance card, photo ID and a list of any current medications to your appointment.
  • Also, due to very limited space, please do not bring non-patients to appointments when at all possible.
  • Any person covered by our work health insurance plan can use any Onsite services without cost.  This includes allergy shots, lab visits, telehealth video visits, infusion services, and services provided by the Onsite Counselor.  
  • Late / No Show Policy: 
    • If a patient does not arrive within 5 minutes of their scheduled appointment time according to the atomic clock, they will be required to reschedule and will receive a $20 NO SHOW / CANCELLATION / LATE FEE. This fee applies even when the appointment is rescheduled to a different time or date, regardless of a telephone call alerting Onsite that the patient will be late for their appointment. This fee will be deducted from the subscriber’s payroll.
    • For disputes regarding late / cancellation fees and general Onsite concerns, please contact Onsite Clinic Manager, Sharla Smith at [email protected].  In her absence, please contact Onsite's Assistant Clinic Manager, Tiffany Hampton at [email protected]. All fee disputes are handled via email only. 
  • Cancellation Policy: 
    • Onsite asks that patients call in order to cancel their appointment at a minimum of TWO hours before the appointment. Failure to call less than 2 hours prior to your appointment will result in a $20 NO SHOW / CANCELLATION / LATE FEEThis fee will be deducted from the subscriber’s payroll. To cancel your appointment you can call the clinic at (931) 906-2001.  For after hours appointment cancellations,  please call (931) 906-2001 and leave a message when prompted in the cancellation mailbox.  Messages left in the cancellation mailbox are time-stamped for verification purposes.
  • Late, Cancellation, or No Show Policy for all visits with the Onsite Counselor / Mental Health Professional:  
    • These visits have a different Late, Cancellation, or No Show policy and fee due to the nature of these appointments.  When a patient is late, cancels, or no shows for these appointments, they have taken a block of time that could be used by someone else.  The service provided by our Mental Health Professionals is valuable and the goal is to reach as many patients as possible. PLEASE NOTE:   A late cancellation of less than 24 hours prior to your appointment will be treated the same as a No Show, thus you will be responsible for the entire NO SHOW / LATE / CANCELLATION FEE OF $40 which will be deducted through the subscriber’s (CMCSS / County employee) payroll. If you arrive more then ten (10) minutes late to your session, it will also be treated as a NO SHOW / LATE / CANCELLATION.  This is necessary because a time commitment is made exclusively for you.  If you are late for a session, you may lose some of that session time.
